Pre-Registering By registering in advance, expectant mothers can be assured that the hospital will have all the basic information it needs before the big day arrives. Pre-admission forms are available in the Admission office. Just complete this form and drop it off at our admitting office during regular business hours, or mail it to the Admissions Department, 128 Lehua Street, Wahiawa, HI 96786. Packing for Your Visit It's a good idea to pack an overnight bag for you and baby a few weeks before your due date and have it handy, just in case you have to be admitted earlier than expected. Here is a suggested checklist of what to pack:
Your Arrival Before leaving the house, please call The Birth Place at 621-4244 so that we can prepare for your arrival. If you are admitted during the day, go to the admitting office in the main lobby to complete the admissions forms. If you come to the hospital after-hours, please go to the receptionist in the Emergency Room (entrance on Kilani Avenue). When you are admitted, you will be asked to sign a Conditions of Admission form and you will them be taken to The Birth Place via a wheelchair. | ||||||||||||||||||||||||||||||||||||||||||||||||
