| Wahiawa Nursing and Rehab Center is a 103-bed ICF/SNF facility. The mission of the facility is to provide care and services to help each resident reach their maximum potential for rehabilitation and to maintain their highest level of self-care. Discharge back to the community or home is the ultimate goal for its residents. Twenty-four (24) hours per day skilled nursing care is provided together with assistance with activities of daily living. To meet individual resident needs, the resident's physician, social worker, dietitian, physical therapist, occupational therapist, and speech therapist work together. |
