The primary goal of this department is to ensure appropriate and individualized continuity of care for patients who require health care services after discharge. The department assists patients and their families in post-discharge planning through assessment, communication and referral to community health agencies, as needed, and to work with other departments to provide optimal, effective discharge planning under a process that is centralized and coordinated. This department is staffed by a Registered Nurse and a Licensed Social Worker.
